List of Documents required while reporting for Counseling:
At the time of counseling, you are required to produce the following documents in original; failure to do so will result in instantaneous cancellation of your claim for admission. You are also required to submit two attested photocopies of each of these documents.

1.     Your letter for Counseling (if counseling letter not received get TAT card.) (If the candidate does not receive the counseling letter he/she should bring TAT card)

2.     For a Proof of date of Birth: SSLC/10th / SSC Certificate or School / College Leaving Certificate or Certificate of Domicile / Nationality Certificate.

3.     Statement of marks of SSLC/X std examination

4.     Statement of marks of PUCII/XII std examination or its equivalent.

5.     Transfer Certificate from the Institution in which you had studied last.

6.     For BRD Category Candidates have to produce, Recent Parent Domicile Certificate, 10 year Study Certificate including (PUC I & II) or (XI & XII).

7.     For KTC Category Candidates have to produce, Service Certificate of the Parent Working in Constituent Colleges of KLE University.

8.     Certificate of registration with state Nursing Council shall be brought by the applicants seeking admission to Post Certificate B.Sc (Nursing) Course.

9.  Applicants seeking admission under SC/ST category shall bring the recent certificate of the same issued by competent authority.

10.  Certificate of Medical Fitness, (as per Annexure -IV).

11.  Recent Conduct and Character Certificate from the college last studied.

12.  An affidavit in the format (as per Annexure-II), signed by you and countersigned by your parent/guardian in the presence of Notary Public on a stamp paper of Rs.50/-

13.  Six recent passport-size colored photographs (as per specification in Rule 58.)

14.  Eligibility Fee:
MBBS & BDS                                                                            Rs.  2,000/-
BAMS, BPT, B. Pharm, B.Sc Nursing & P.C.B.Sc Nursing            Rs.    500/-

15.  DD’s (of Nationalised Bank) for Tuition fees.

16.  DD’s (of Nationalised Bank) has per fees structure should be drawn in favour of :"Registrar KLE University" payable at Belgaum

17. For paying tuition fee and eligibility fee two separate demand drafts from any nationalized bank shall be drawn in favour of Registrar, KLE University payable at Belgaum.
