1. Name of the course: Certificate Course in Periodontal Esthetic & Plastic Surgery - Application Form
2. Preamble:
This modular program will provide the student an in-depth experience with the gamut of esthetic and reconstructive surgeries. The course will highlight the use of a variety of osseous grafts and soft tissue grafts for predictable coverage of lost periodontal tissues due to disease processes.
3. Duration: Six months consisting of 06 modules each of 03 days duration conducted every month
4. Eligibility: B.D.S. degree recognized by DCI. or M.D.S.
5. Total Intake: 10 students.

7. Last date to receive (by post) down-loaded and completed application form by the University along-with Demand Draft (drawn on any nationalized Bank) for Rs-500/- favouring Registrar, KLE University, payable at Belgaum is 26th July 2008 at 5.00 PM.
8. Selection/Admissions will be based on test/interviews conducted by the KLE University, Belgaum.
9. Date for Entrance Test/ Interviews is on 30th July 2008.
